Wilos In Home & Community Care are a dedicated Aged Care Coordinator & Service Provider that has partnered up with Trilogy Care. Together we are committed to supporting elderly individuals through a range of compassionate and personalized services. Providing services across Western Melbourne Suburbs, Regional Western Victoria, Greater Geelong, and Surf Coast areas.

To take comfort in knowing all our carers are qualified and Police checked to assist you with a range of services. By allowing our clients to take full control over their Home Care Package with the choice of two self-management options saving your funds to access more services.

THE ROLE OF WILOS IN HOME AND COMMUNITY CARE
Wilos in Home & Community Care provides various services that can be funded under your
Home Care Package – subject to your individual care plan and allocated care budget.
We partner with Trilogy Care, a registered Home Care Package provider, who can help you
self-manage your Home Care Package.
One of the benefits of self-management is the freedom to choose your own care workers and
service providers. This means you can continue to work with Wilos in Home & Community
Care while having your services funded under your Home Care Package budget.
Working with Trilogy Care means you will spend less on Home Care Package management
fees so you can spend more of your Home Care Package budget on the things that matter –
care and services to help you stay at home longer.

 

WHO IS TRILOGY CARE


Trilogy Care supports older Australians to live independently at home for longer by helping
them build and self-manage the Home Care Package they want and need.
The experienced Trilogy Care team works with Wilos in Home & Community Care to
maximize your care hours and wellbeing, giving you choice, control, and independence.

 

 

 

 

 

WHAT OUR PARTNERSHIP OFFERS

SELF MANAGEMENT is ideal for people who want to manage their Home Care Package themselves or with the support of family.

FULLY COORDINATED  you are provided with care coordination. Fully  Coordinated is ideal for individuals who want the benefit of self-managing, with extra help from a care coordinator throughout the year.

Self Management and Fully Coordinated are both charged under a transparent fixed fee structure currently 15% for Self-Managed and 26% for Fully Coordinated with no setup fees, no daily fees, or exit fees. This means more of your Home Care Budget is available for care and services, not provider fees.

What is included in the Trilogy Care Self Managed or Fully Coordinated fee?

  • Care management. A dedicated care partner for every care recipient. The design and ongoing maintenance of your care recipient's care plan and care budget including periodic care plan reviews
  • Package management, Management of the financial components of the Home Care Package, including payment of invoices, reimbursements, monthly statements, and liaising with Services Australia.
  • Clinical support, Phone access to our in-house team of clinical nursing professionals to oversee the care recipient's care plan and any health concerns
  • Compliance support the compliance and background checking of all service providers
  • Care coordination – FULLY COORDINATED only. If a care recipient opts for Fully Coordinated, Trilogy Care’s internal care coordination team will support the care recipient through their Home Care Package journey.

At Wilos in home & Community Care we provide comprehensive support to individuals requiring assistance with their day-to-day activities to live independently in their own homes. This includes full or partial help with bathing, personal grooming, using the restroom, getting dressed, and managing medications.

 

 

Meal Preparation

Our Meal Assist service is designed to help clients who may be unable to cook for themselves or who simply require some help in meal preparation. This can include assistance with breakfast, preparing sandwiches for lunch, or chopping and readying vegetables for dinner.

 

Home care

Basic Home Cleaning services:

  • vacuuming, mopping, dusting
  • cleaning bathroom, toilet & kitchen
  • Laundry & change of bed linen

We also provide Spring clean service this is a more detailed service.
